The completion of specific information returned to HEC is mandatory for your child to begin participation in the education programs offered at our special education school. The following forms must be completed and returned:
• Registration Information (if required by school district)
• Immunization Record
• Emergency Information Card
• Emergency Health and Safety Parental Permission and Release
• Student Health/Medical Standing Orders
• Medication Dispense Release
• Transportation Information (if child is transported by HEC)
• Transportation Contract
• Off-Grounds Permission Slip
• Application for Free or Reduced-Price Meals
• Child Abuse Information
• Social Security Number
• Change in Residence Notice
